We think we have outlined in this book a mature technical description for the usage of drugeluting stents. Despite all the worrisome information about drug-eluting stents, I remain convinced that these devices represent a major advance in the treatment of coronary artery disease. Even though the principles of evidencebased medicine must remain second to providing hard data, drug-eluting stents are saving lives! Perhaps the problem is that these devices are such powerful tools that we are often so enthused by the lovely angiographic result that we do not give sufficient attention to proper patient selection or to optimal stent implantation.
We are now performing procedures with drugeluting stents that we never dreamed of doing a few years ago. The low late loss is sometimes used as justification for accepting sub-optimal results, and this may be one of the problems. Instead of defining how drug-eluting stents should be implanted, in this book we are trying
to give some direction. I write this preface when the book is nearly in print and already I would like to add new information and make additional changes. If I can but highlight two important points this book hopes to make, I would stress the value of optimal stent deployment and the value of intravascular ultrasound. Maybe it is
now time to begin another book about optimal drug-eluting stent implantation with intravascular ultrasound guidance.


I can already imagine how excited Goran Stankovic will be about this new task, only a few weeks after having completed the current book. More importantly I would like to express my thanks to all the contributors who worked so hard and productively to deliver all the chapters in time. We all know how easy is to say yes when a colleague asks about writing a chapter in a new book and how demanding it is to realize that the deadline is looming and so much work still needs to be done. All the co-authors have been
able to meet the deadline and to deliver enjoyable and useful write-ups. I am hopeful everybody will find the time spent reading this book a useful experience and
I hope many patients will benefit from the efforts all the contributors have put into this work.
